RN Case Manager
Full-time professional position to assess, admit and provide case management for hospice patients. This position works to provide optimum patient comfort as part of an interdisciplinary team under the attending physician's order consistent with the agency's mission statement and regulatory systems; Registered Nurse with current NC license; BSN preferred; Minimum 2 years recent clinical experience; Hospice and/or home health or nursing home experience or equivalent preferred; Hospice Nurse Certification preferred; Must have access to transportation.
Home Health RN Case Manager
This is a full-time professional position that will provide case management for Home health patients assigned; providing optimum patient comfort as part of an interdisciplinary team under the attending physician's orders consistent with the agency's mission statement and regulatory systems; Registered Nurse with current NC license; BSN preferred; Minimum 3 years recent clinical experience; Minimum 2 years home health and/or hospice experience or equivalent; Maintain basic CPR certification to meet Nurse Practice Standards; Knowledgeable about reimbursement source, Medicare regulations, licensing laws, and accreditation standards; Computer skills preferred including knowledge of Oasis; Must have access to transportation.
PRN LPN
This is a per diem professional position. This position provides scheduled visits for patients assigned; providing optimum patient comfort as part of an interdisciplinary team under the RN Case Manager following the attending physician? orders consistent with the agency? mission statement and regulatory systems; Licensed Practical Nurse with current NC license; Minimum 2 years recent clinical experience. Hospice and/or home health or nursing home experience or equivalent preferred; Hospice Nurse Certification preferred; Knowledgeable about reimbursement sources, Medicare regulations, licensing laws, and accreditation standards; Competent computer skills; Must have reliable transportation.
